To make your registration at Brunswick Community Hospital as easy as possible, we remind you to bring:
- The orders from your physician
- Your insurance cards
- A photo ID
- Your Living Will, if you have one
- Your Durable Power of Attorney, if applicable
- You may be asked to sign a Consent to Treatment.
- May be asked to sign an Authorization to Bill Insurance.
- If you're a Medicare recipient, you'll be asked to complete a Medicare Rights Statement and a Medicare Secondary questionnaire.
